

Register Implants
Registering your implants is for your own peace of mind and years of confidence in your
decision. You have a 90-day window after surgery to do it. We’ll walk through it with you!

Things to keep in mind
- Implant registration must be done by the patient
- This process is solely applicable to Motiva® Implants
- They must be registered within 90 days of your surgery to be valid
- It has no cost
- To purchase an extended warranty, your Motiva® Implants must be correctly registered first
The “How-to”
1. Sign-up/ Sign-in to the MotivaImagine® App
Open the MotivaImagine® website and sign up with either your Facebook or Google account or with your email address. Fill in your Personal Information as requested.


2. Review and Accept the Terms & Conditions
Read and accept the Terms & Conditions by checking the boxes you agree to and click “Submit”.
3. Register Your Implants
From the Home page click “Register my implants”, select your registration method (with SN or ESN), and then complete the implant and surgery information required in each step.


Registration Confirmation
You will receive an e-mail confirming that your implant(s) have been registered successfully.
